GENERAL General requirements for admission include:

APPLICATION PROCEDURES

Make an appointment with the admissions director to tour the residence. |

Fill out an application and return it to the admissions director. |

A request for information will be sent to your personal physician to fill out a form detailing your health
history. A complete physical examination within 30 days prior to application is necessary.
