COMPLEX MEDICAL/POST–SURGICAL CARE
We provide transitional and long-term care to people with complex medical conditions
that require medical management. Most frequently, people requiring complex medical
care are recovering from a stroke, heart attack, or surgery. Or they
need help managing the effects of degenerative conditions like Parkinson's disease.
Our services concentrate on physically reconditioning the patient. We work toward increasing
the patient’s exercise tolerance by strengthening essential muscle groups, improving the patient’s
oxygen use, and enhancing cardiovascular response to physical activity.
We provide services such as:
| - | Insertion and Maintenance of peripheral IV lines |
| - | Care and Maintenance of central IV lines |
| - | Oxygen therapy and management |
| - | Gastrostomy tube (“g-tube, “feeding tube”) care and management |
| - | Wound Care |
Along with these medical services, we also educate patients about how to manage their condition
successfully when they return home after a short – term rehabilitative stay.

HOSPICE-RELATED SERVICES
Hospice programs offer a combination of home and facility care for terminally ill patients
and support for their family members. Hospice services focus on relieving pain and
symptoms ("palliative care") rather than on seeking a cure. The goal is to help a terminally
ill person live his or her last days with as much dignity, comfort, and enjoyment as possible,
at home or in a homelike setting. At the Stone Institute, we provide these services through an
approved hospice program.

PAIN MANAGEMENT
Pain management services are needed by people who are recovering from surgery or a
fracture, living with chronic pain from a condition such as arthritis, or terminally ill.
For patients in the first two categories, we help them manage their pain with medication.
We closely monitor and manage medication dosages and effectiveness. The goal is to
find the lowest effective dosage that affords the patient pain relief. This is a process of
continual assessment and adjustment.

RADIOLOGY AND LABORATORY SERVICES
The Stone Institute schedules appointments and coordinates arrangements with
appropriate outside providers for any x-ray, blood, or other lab tests ordered by
a physician for our residents.

RESPITE CARE
Respite care is short-term residency for people whose primary at-home caregivers:
Are temporarily unavailable (on vacation, on business, or ill)
Just need a little time off from the responsibility of full-time care of an older adult
During a respite-care stay, residents receive the same high-quality care as our other residents,
in a safe and secure environment. Respite-care residents may enjoy all the services and
programs we offer during their stay. Respite-care stays may be arranged for as long as
necessary—a weekend, a week, a month, or more.

RESTORATIVE PROGRAMS
The Stone Institute's Restorative program relies on rehabilitation techniques to prevent
disability and promote ability in our residents. Restorative care is appropriate for residents
who no longer need intensive physical therapy services, but who still need some rehabilitation
to reach their optimal level of independence of to maintain the progress they’ve already made.
Our Restorative program is a team effort, at the core of which are our nursing staff and
physical therapists. Together, they deliver care that maintains or improves a resident's
functional abilities in an environment that is positive, cheerful, caring, and patient.
Patience is particularly important because by allowing a resident sufficient time to
perform activities, the caregiver is also helping preserve the resident's independence
and sense of self-worth.

SKIN INTEGRITY MANAGEMENT
The Stone Institute's Skin Integrity Management program has two distinct goals:
To prevent infection and promote healing of existing wounds (surgical wounds;
pressure ulcers; or other chronic, non-healing wounds)
To prevent the breakdown of currently healthy skin
In order to attain the program's goals, we provide the following services:
Pressure relief management
Nutrition and hydration management
Education and training on stomal care
Wound rounds
Maintaining healthy skin is a constant challenge with many nursing home residents.
Because of this, a very important part of our Skin Integrity Management program is
to identify other health issues that might increase the chances that a resident's currently
healthy skin might break down in the future. These risk factors might include a patient's
lack of mobility, incontinence, or inadequate nutrition. Once we identify these factors,
we address them in the patient's individualized care plan.

REHABILITATION SERVICES
The Stone Institute’s rehabilitation team develops an individualized plan of care designed to
meet each resident’s specific goals and expectations.
We are proud to offer rehabilitative services 7 days per week to maximize individual resident’s
progress and recovery. Our therapists are all employees of The Stone Institute, which ensures
continuity of care. The staff takes pride in the progress they see in each individual resident
on a daily basis.

PHYSICAL THERAPY
Physical therapy is the treatment of injuries or medical conditions by using physical methods.
Some examples of these methods are exercise, massage, and the application of heat or ice. Physical therapy services begin with an evaluation
of the patient's strength, coordination, flexibility, balance, and mobility. Then, the therapist
devises a program to help the patient regain as much physical function as possible.

OCCUPATIONAL THERAPY
Occupational therapy helps a person relearn everyday physical skills that were lost due
to an illness or injury. Occupational therapy focuses on the activities of daily living—
bathing, grooming, toileting, dressing, eating, and housekeeping. An occupational
therapist evaluates a patient to determine how that person's thinking abilities,
limb movement, coordination, mobility, balance, and vision are affecting his or her
ability to perform daily tasks. Then, the therapist provides education and training that
helps make these activities easier to perform. The goal is to help the person live with
as much independence as possible.

SPEECH THERAPY
Speech therapy is used to help a person work through verbal communication problems.
Therapy depends on whether the communication problem results from a physical or
intellectual impairment. It may include exercises to strengthen either the muscles
that help produce speech or the connections between thoughts and verbal communication.
